Athletic Clearance

As required by CIF Bylaw 503G, all student-athletes are required to receive an annual preparticipation physical examination (PPE) certifying that the student-athlete is physically fit to participate in athletics. The physical must be completed and turned into Home Campus on the school-approved Preparticipation Physical Examination Form prior to any tryout, practice or interscholastic athletic competition. All physical examinations must be completed and signed by an MD, DO, PA, or NP. We are unable to accept any physical examination signed by a DC (Doctor of Chiropractic).

Additionally, all incoming and returning student-athletes are required, on an annual basis, to complete the online athletic clearance prior to any participation in sports. This can be completed at www.homecampus.com.

aTHLETIC cLEARANCE iNSTRUCTIONS

In order to participate in athletics at RMHS you must first be athletically cleared which involves the following:

*Creating an account  at www.homecampus.com/login

Any returning athletes may sign in with the same information. Otherwise, create an account and complete the registration page. 

Once logged-in click on the “Start Clearance Here” button on the right side of the screen.

On the next screen type in Rio Mesa as the school, select the school year, and add every sport your child may potentially play.

*Complete all consecutive screens

*Must have medical insurance & provide name and ID or policy number on medical card

All physicals must be done on the form provided in the link below. 

Physicals are required to have an exam date listed and stamped by the healthcare provider (with an exception of Kaiser Permanente only, who provides their own form for sports physicals).

Physicals turned in without this information will be denied clearance until corrected. 

Physicals will only be accepted through homecampus.com. Please upload all pages of the packet. No paper copies need to be turned in or will be collected by the school.

Once all steps are completed you will receive an automated email from Homecapus.com, this does not mean the student-athlete is cleared to participate in activities/athletics, only that the clearance is submitted for review. 

Please allow 1-3 days for processing. You will receive an email once the student-athlete is cleared.

Physical Day

Every year RMHS has doctors come to the school to provide physical examinations at a reasonable price. An athlete may either go to their own physician or attend the physical day on campus. If both are unavailable, reach out to the athletic trainer for other options.
